LOUISVILLE, Ky. — With flu, COVID-19 and other respiratory viruses circulating, Louisville health officials are reminding residents to review their vaccination history to ensure they’re up to date.

The Louisville Metro Department of Public Health and Wellness said the best place to start is with the health care provider or clinic that administered the vaccine. Patients whose providers use MyChart can view and download vaccination records through that system.

Those who received vaccines at a pharmacy can request records directly from the pharmacy. Official immunization records — for adults or children — are also available through the Kentucky Immunization Registry via its public portal.

The health department noted that insurance companies may be able to identify where a vaccine was given by reviewing claims data, but those records are not considered official proof of vaccination.

Officials said vaccination needs vary by individual. Factors such as age, vaccine type, underlying health conditions and medical history can affect eligibility and recommendations.

The department also operates an Immunizations Clinic for people ages 2 months and older, including adults, to get caught up on routine vaccines. Appointments can be scheduled by calling (502) 574-5380.

Residents visiting the clinic should bring or know their vaccination history. Official records must include details such as patient information, vaccine manufacturer, lot number, date of administration and the clinic site or provider’s signature.
