Michael Shawn Boaz pleaded guilty to conspiracy to commit health care fraud and issuing fraudulent prescriptions.
PADUCAH, Ky. — A Kentucky pharmacist was sentenced to prison Friday and ordered to pay more than $1.8 million in restitution.
On Nov. 3, 2025, 47-year-old Michael Shawn Boaz of Clinton pleaded guilty to conspiracy to commit health care fraud and fraudulent prescriptions.
“Fraudsters like Boaz that steal from health care benefit programs negatively impact everyone’s health care costs. Health care fraud is a top priority of our office,” said United States Attorney Kyle Bumgarner. “We will continue to aggressively investigate and prosecute those that defraud our health care programs. It is my fervent hope that Boaz’s prison sentence serves as a strong deterrent to other fraudsters in the Western District of Kentucky to stop their fraud now because we will find you and prosecute you.”
Court documents showed Boaz and a co-conspirator fraudulently billed health care benefit programs for customers at two Kentucky pharmacies while working as licensed pharmacists, prosecutors said. Between 2017 and 2021, they caused nearly $2.2 million in losses to benefit providers by submitting claims for fraudulent and unauthorized prescriptions.
Boaz added “boutique vitamins” to customer prescription orders and told them they were “free,” prosecutors said. However, customers were billed between $1,000 and $6,000, according to prosecutors. He also submitted claims for the antihistamine Carbinoxamine Maleate.
Prosecutors said Boaz purchased Carbinoxamine Maleate from a wholesaler for $605 per prescription and billed health care providers between $2,000 and $16,000 for each fraudulent prescription.
Boaz was sentenced to 30 months in prison, followed by three years of supervised release.
